From Southend University Hospital and NHS South East Essex. The PCT has commissioned a further extension of the COPD community scheme. This will provide consultant-delivered outreach clinics for Respiratory Medicine in South East Essex. These will be locality based to cover the practice-based commissioning consortia areas bringing patient care closer to home.
This service will also offer primary and community service providers the opportunities for clinical leadership, case management via Multi-Disciplinary Team (MDT) meeting and virtual clinic. This will improve the respiratory health of the local community in particular supporting care for patients with chronic respiratory disease with the aim of reducing A&E attendance, admissions, length of stay and will contribute to a shift of hospital outpatients activity to a community setting.

Discussion paper from IMPRESS to inform the implementation of the Strategy for Services for Chronic Obstructive Pulmonary Disease (COPD) in England, the Service Framework for Respiratory Health and Wellbeing in Northern Ireland, Service Development and Commissioning Directive (Chronic Respiratory Conditions) in Wales and the Clinical Standards programme (COPD) in Scotland within the context of limited resources.
Given current policy imperatives, IMPRESS asked the members of its two founding clinical societies, the British Thoracic Society (BTS), and Primary Care Respiratory Society UK (PCRS-UK) for practical examples of opportunities to improve quality and reduce cost. This paper summarises some of the findings about optimising smoking cessation, rationalising oxygen use and other prescribed medicines, supporting patient adherence, and improving use of hospital beds and specialist advice. We have included examples that we believe exemplify good practice.

Other examples - Lung Improvement Programme (LIP) and NHS Evidence
From January 2010 the Lung Improvement Programme (LIP) conducted a search for examples that "promote high standards in efficiency and effectiveness, as well as providing good quality integrated care across all stages of severity and have clearly measurable outcomes against a robust baseline of activity" in COPD and asthma. TThey requested a covering email giving permission for the example to be shared across the NHS.
May 2010: here are the first tranche of examples, categorised by theme and by SHA.
Where appropriate, examples will be shared with the Department of Health which is supporting the collection of specific examples of good practice to be published on the NHS Evidence web-site.
June 2010 National Improvement projects in England - pilots announced by Lung Improvement following calls for pilots in each region on these topics (those with asterisk tackling the whole care pathway).
