This document provides a review of evidence for recommended clinical interventions by Dorset's respiratory strategy group, an analysis of five English integrated respiratory models tailored to managing people in their own home, and a review of existing community-based respiratory services in England focused on COPD, bronchiectasis and pulmonary fibrosis to support the development of an integrated respiratory model for Dorset.

A new LES from Islington, that complements the COPD LES year 4, to incentive stop smoking support in general practice.

This paper was commissioned by the Integration workstream of the NHS Future Forum. It grew from a section of National Voices’ Principles of Integration (2011), which was developed in close consultation with member charities.

The specific needs of the three exemplar groups were discussed with Mind, Rethink Mental Illness and the National Children’s Bureau (all members of National Voices) and with Age UK. Angela Coulter, a National Voices trustee and foremost authority on patient experience and involvement, commented and quality assured the draft.
